Job Description

Augusta University Health System is seeking a dedicated and compassionate Social Worker to join our dynamic team. The successful candidate will play a crucial role in supporting patients and their families by assessing psychosocial needs, providing counseling, facilitating access to community resources, and advocating for patient rights. Key Responsibilities

  • Conduct comprehensive psychosocial assessments of patients and families to identify needs, strengths, and resources.
  • Develop and implement individualized care plans in collaboration with patients, families, and the multidisciplinary healthcare team.
  • Provide counseling, crisis intervention, and support to patients and families coping with illness, trauma, loss, and challenging life circumstances.
  • Facilitate referrals to appropriate community resources, support groups, and external agencies to ensure continuity of care.
  • Advocate for patients' rights, preferences, and access to necessary medical and social services.
  • Educate patients and families on disease management, treatment options, navigating the healthcare system, and advance care planning.
  • Participate actively in multidisciplinary team meetings, patient care conferences, and contribute to effective discharge planning.
  • Maintain accurate, timely, and confidential documentation of all patient interactions, assessments, interventions, and care plans in electronic health records.
  • Adhere to all professional ethical standards, organizational policies, and relevant federal, state, and local regulations pertaining to social work practice and patient confidentiality.

Required Skills

  • Master's degree in Social Work (MSW) from an accredited program
  • Current licensure or eligibility for licensure as a Social Worker in the State of Georgia
  • Minimum of 2 years of post-MSW social work experience, preferably in a healthcare setting
  • Demonstrated ability to conduct thorough psychosocial assessments and develop effective care plans
  • Strong crisis intervention and advanced counseling skills
  • Excellent written and verbal communication and interpersonal skills
  • Ability to work effectively and collaboratively within a multidisciplinary team environment
  • Cultural competence and sensitivity to diverse patient populations and backgrounds
  • Proficiency in electronic health record (EHR) systems and standard office software

Preferred Qualifications

  • Experience with medical social work in an academic medical center or large hospital system
  • Fluency in multiple languages, particularly Spanish
  • Experience in a specialized area such as oncology, pediatrics, emergency medicine, or critical care
  • In-depth knowledge of local community resources and social service networks in Augusta, Georgia
