Job Description

We are seeking a compassionate and dedicated Community Nurse to join our team in Garland, Texas. This role is pivotal in delivering high-quality, patient-centered care within a community setting. The Community Nurse will be responsible for assessing, planning, implementing, and evaluating nursing care for individuals and families in their homes or designated community locations. This position requires strong clinical skills, excellent communication, and a commitment to promoting health and well-being across diverse populations.

Key Responsibilities

• Conduct comprehensive health assessments, including physical, psychological, and social factors, for patients in community settings.
• Develop, implement, and evaluate individualized care plans in collaboration with patients, families, and interdisciplinary team members.
• Provide skilled nursing care, including medication administration, wound care, ostomy care, and other treatments as prescribed.
• Educate patients and their families on disease management, health promotion, medication adherence, and self-care techniques.
• Monitor patient progress, identify changes in health status, and report findings to the appropriate healthcare providers.
• Advocate for patients' rights and facilitate access to necessary community resources and services.
• Maintain accurate and thorough electronic health records and other required documentation.
• Collaborate with physicians, social workers, therapists, and other healthcare professionals to ensure coordinated care.
• Participate in community health initiatives, screenings, and educational programs as needed.
• Adhere to all professional nursing standards, ethical guidelines, and agency policies and procedures.

Required Skills

• Valid Texas Registered Nurse (RN) license in good standing.
• Current Basic Life Support (BLS) certification.
• Minimum of 2 years of experience as a Registered Nurse, preferably in a community, home health, or public health setting.
• Strong clinical assessment and critical thinking skills.
• Excellent verbal and written communication abilities.
• Ability to work independently and manage a diverse caseload.
• Proficiency in electronic health record (EHR) systems.
• Valid driver's license and reliable transportation.

Preferred Qualifications

• Bachelor of Science in Nursing (BSN) degree.
• Certification in Community Health Nursing or a related specialty.
• Experience with diverse patient populations and cultural competency.
• Bilingual proficiency (especially Spanish) is a plus.
